Research Progress on External Treatments of Lumbar Disc Herniation in Traditional Chinese Medicine

Abstract：
Through literature review and analysis,
this article describes the research progress of external Traditional Chinese
Medicine (TCM) treatments for lumbar disc herniation (LDH), covering etiology
and pathogenesis, acupuncture, massage (Tuina), and external application of
Chinese herbal medicines. It also summarizes the mechanisms of action to
provide references for clinical LDH management. The pathogenesis of LDH in TCM
is related to improper work and rest, aging, and irregularities in daily life
that damage the liver, kidney, qi, blood, tendons, and bones. Various external
TCM treatment methods are used clinically for LDH, such as acupuncture,
massage, herbal application, fumigation, and moxibustion, often in combination.
These methods work synergistically to reduce inflammatory factor levels,
eliminate tissue adhesion, improve lumbar function, decrease the degree of
lumbar fibrosis, improve neuroelectrophysiological parameters, and reduce the
recurrence rate. Their mechanisms of action are associated with the regulation
of inflammation, oxidative stress, extracellular matrix metabolism, immune
cells, pain factors, and nerve conduction velocity.

摘要：通过文献阅读与整理，从病因病机、针刺、推拿、中药外用等方面阐述腰椎间盘突出症的中医外治法研究进展，并总结其作用机制，为腰椎间盘突出症的临床治疗提供参考。腰椎间盘突出症病机与人体劳逸不当、年龄增长、起居失常损伤肝肾气血筋骨有关。中医临床治疗腰椎间盘突出症外治方法多样，如针刺、推拿、中药敷贴熏蒸、艾灸等相互结合，共同发挥降低炎症因子水平、消除组织粘连、改善腰椎功能、降低腰椎纤维化程度、改善神经电生理、降低疾病复发率等作用。其作用机制与调节炎症、氧化应激、细胞外基质代谢、免疫细胞、疼痛因子、神经传导速度水平有关。
